Web to Lead Form Activating more than one Custom field

Good Day,

We have created Web to lead forms for our website, and in each form we have entered one custom field depending on which event they are interested,  however when a client ticks the custom field or submits the form it ticks all the custom fields triggering the incorrect workflows.

Please can you help?
